Study Shows Benefits of Bariatric Surgery for People With Diabetes

A new study investigating bariatric surgery outcomes for obese individuals with diabetes has just been published in the Journal of the American Medical Association. The results may be shocking to some, but likely not so shocking for bariatric surgeons and their weight loss surgery patients.

The study looked at 2,287 adults with obesity and diabetes who underwent either gastric sleeve, gastric bypass, gastric banding, or duodenal switch and 11,435 control patients who had diabetes and obesity but received standard medical nonsurgical care. The study was done through the Cleveland Clinic Health System between 1998 and 2017, and participants in the two groups were balanced by sex, age, BMI, and glycated hemoglobin (which is a measure of diabetes control).

The results show the many benefits bariatric surgery can have for people with obesity and diabetes. The following is a summary of the findings after 8 years:

  • Surgical patients had a mean body weight loss of 29.1 kg (64 lb) compared to nonsurgical patients 8.7 kg (19.1 lb)
  • Significant reduction in HbA1c of surgical patients compared to nonsurgical patients
  • Significant reduction in noninsulin diabetes medications, insulin, renin-angiotensin system blockers, other antihypertensive medications, lipid-lowering therapies, and aspirin compared to nonsurgical patients
  • Major Adverse Cardiovascular Events were significantly lower in the surgical group (31%) compared to the nonsurgical group (48%)
  • All cause mortality, coronary disease events, heart failure, atrial fibrillation, cerebrovascular events, and nephropathy were all lower in the surgical group versus the nonsurgical group
